Reporting Requirements

The information located on this page is for currently funded and active D-CFAR and CFAR sites.

Progress Report Guidelines

Please use these guidelines and spreadsheets when submitting your annual progress reports. Progress reporting guidelines (Word)

Please also provide the following Tables in the annual progress reports.

• Previous Recipients of Developmental Awards Table (Excel)
• Existing Core Facilities at the Institution Applying for a CFAR Award (Excel)
• Core Utilization Table (for all Basic and Clinical Cores) (Excel)

Information required on an ongoing basis

During the budget period, some information may be needed after each cycle of awards made at the CFAR site. Please see the information below to determine what information needs to be submitted.

• Current Developmental Awards
• Please complete and submit a Current CFAR Developmental Awards table after the completion of each developmental award cycle. Please contact Ann Namkung Lee for a copy of the excel spreadsheet template

Additional NIH approval may also be needed before initiation of a project and before CFAR funds are disbursed.

• Studies with an international component
• Please use this checklist form when submitting any international studies for clearance. International Studies Checklist (for all CFAR-Supported International Studies) (Word)
